WAIVER AND RELEASE OF LIABILITY/MEDICAL PERMISSION/SAFETY
In consideration of SSPP furnishing services and /or equipment to enable me to participate in paintball games, I agree as follows: I fully understand and acknowledge that; (a) risks and dangers exist in my use of Paintball equipment and my participation in Paintball activities; (b) my participation in such activities and /or use of such equipment may result in my injury or illness including but not limited to bodily injury, disease strains, fractures, partial and /or total paralysis, eye injury, blindness, heat stroke, heart attack, death or other ailments that could cause serious disability; (c) these risks and dangers may be caused by the negligence of the owners, employees, officers or agents of SSPP; the negligence of the participants, the negligence of others, accidents, breaches of contract, the forces of nature, or other causes. These risks and dangers may arise from foreseeable or unforeseeable causes; and (d) by my participation in these activities and /or use of equipment, I hereby assume all risks and dangers and all responsibility for any losses and /or damages, whether caused in whole or in part by the negligence or other conduct of the owners, agents, officers, employees of SSPP, or by any other person. I, on behalf of myself, my personal representatives and my heirs, hereby voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify SSPP and it’s owners, agents, officers, and employees from any and all claims, actions or losses for bodily injury, property damage, wrongful death, loss of services or otherwise which may arise out of my use of Paintball equipment or my participation in Paintball activities, I specifically understand that I am releasing, discharging and waiving  any claims or actions that I may have presently or in the future for the negligent acts or other conduct by the owners, agents, officers, or employees of SSPP.

MEDICAL PERMISSION AUTHORIZATION     If the participant is of minority age, the undersigned parent or guardian hereby gives permission for SSPP to authorize emergency medical treatment as may be deemed necessary for the child named below while participating in paintball games from this date through 12/31/2010.

Video and Picture Authorization & Release    The undersigned parent/guardian/player hereby releases all claims and rights to any and all pictures/video taken by SSPP for it's use or authorized use by SSPP for it's purposes.

SAFETY     Goggles must be worn at all times while on the field, at the chronograph, and target range. Violators will be warned and can be removed from the playing field without a refund. Check your issued goggles  for a snug fit and any damage. There will be no altered goggles from its’ original manufactured state, (missing face and ear protection), allowed on the field. Barrel sleeves must be on the paintball guns at all times except when player is shooting at the chronograph, target range, or actually playing in a game.

Do not shoot the paintball gun unless you are in a game, at the chronograph, or at the target range. Do not shoot at any animals, people, or objects outside of the playing field. You cannot jump or climb on/over, aggressively touch any bunker, climb trees or any unauthorized structures. Do not cross the designated boundaries- mowed lines between fields, the fences, and any rope border designating out of bounds. Crossing these boundaries is an instant elimination. Do not shoot any players within 15’ without first calling for “surrender”, twice loudly and clearly”. Surrender is "optional", if you want to surrender, the player must raise both arms above their head and repeatedly say, “surrender”, otherwise the engagement continues. Do not shoot for the head, but for the body. Do not shoot a player repeatedly once they have been hit. All markers must be chronographed (3 shots) under 290 fps. safety limit before it may be used. If a field judge suspects a marker has been tampered with, he may remove that player and marker for testing at the chronograph. Players are not permitted to bring velocity-adjusting tools onto the field; any player caught adjusting or altering their marker so as to affect the velocity of the marker, will be removed from the playing field for the rest of that session (4 hours) without a refund. There will be no alcohol, drugs, firearms, knives, physical contact, or abusive language allowed on this field. Violation of any of these offenses will result in the removal of the offending player (s) from the field. Obey the field judges at all times. Report any hazards, injuries, unsafe equipment, and abusive or unsafe players to any field judge.

I HAVE READ THE ABOVE WAIVER AND RELEASE AND SAFETY RULES AND PLAYER CONDUCT AND BY SIGNING IT AGREE IT IS MY INTENTION TO EXEMPT AND RELIEVE SSPP FROM LIABILITY FOR PERSONAL INJURY, PROPERTY DAMAGE OR WRONGFUL DEATH CAUSED BY NEGLIGENCE OR ANY OTHER CAUSE.     PLEASE FILL OUT COMPLETELY
